Privacy Policy
This policy explains how Good Practice (GP) Surgery handles personal information, including health information, in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
Information we collect
We collect personal information that is necessary to provide you with health care. This may include your name, date of birth, address, contact details, Medicare and health fund details, emergency contact, and health information such as your medical history, medicines, allergies, test results and consultation notes.
How we collect it
Wherever practical we collect information directly from you: in person, over the phone, through registration forms or through our online booking provider. We may also receive information from other health providers involved in your care, such as hospitals, specialists, pathology and imaging services, or from your previous general practice with your consent.
How we use and disclose it
Your information is used to provide and manage your health care, to communicate with you about appointments, reminders and recalls, for billing and Medicare claiming, and to meet our legal and accreditation obligations.
We may disclose your information to other health professionals involved in your care, to Medicare or your health fund for claiming, and where disclosure is required or authorised by law. We do not sell personal information, and we do not disclose it for marketing purposes.
Storage and security
Records are held electronically and, where relevant, in paper form. We take reasonable steps to protect information from misuse, loss and unauthorised access through access controls, staff confidentiality obligations, secure systems and secure destruction of records that are no longer required to be kept.
Access and correction
You may request access to the personal information we hold about you, and ask us to correct it if it is inaccurate or out of date. Requests should be made in writing to your clinic. In limited circumstances we may need to decline a request, and we will explain why in writing.
Cookies and website analytics
This website may use cookies and basic analytics to understand how pages are used and to improve the site. This information is aggregated and does not identify you personally. You can disable cookies in your browser settings, though some parts of the site may not work as intended. Our online booking provider operates its own website and privacy policy, which applies when you follow a booking link.
